Human error is reported to be relevant to incident in chemical process industry more than 70% directly or indirectly. Therefore, introducing an appropriate tool to analyze and quantify human error in chemical process industry is becoming more and more important. A brief introduction on how to analyze human error will be made and approach on how to apply it to reduce and prevent loss in chemical process industry will be introduced. |
