제목 A Study on the Root Cause Analysis based on Humanities and Social Sciences related to Cracked-Rupture Accident of Sulfuric Acid Coolers Occurred in Initial Startup of Sulfuric Acid Plants
초록 This paper is to search for root/contributing causes against the cracked-rupture accident of sulfuric acid coolers in the chemical process that occurred in initial startup of sulfuric acid plants for performance guarantee test run. Because of unknown origin on these accidents, these plants were shutdowned at short intervals. In consequence, these accidents had been calling the contamination of cooling water by sulfuric acid leaking from this accident, and also process equipment on cooling unit were corroded. Hydrogen gases were started to emit around cooling tower unit. At last, these gases were exploded every maintenance works were done on cooler units. So, all core factors for trouble-shooting on the above is analysed why such accidents periodically have been occurred in the chemical process system. Accordingly, this study is searching to find out the root/contributing causes on this matter and showing what and how to overall design on this complex chemical plants.
